From nurses to cleaners
January 25, nurses from the Kolomna Perinatal Center (KPC) in the Moscow region published a letter to the president, head of government, minister of health, governor of the Moscow region, prosecutor, deputies and the media. They announced that they would fast one after the other with an interval of five days. On February 2, junior nurse Natalya Trukhina began a hunger strike while working a 12-hour shift. In total, 11 junior nurses signed the letter.
In the letter, the nurses complained about unbearable working conditions. As one of the employees who will take part in the hunger strike, Ilona Rusotskaya, told The Insider, there have been two waves of layoffs in the maternity hospital. The nurses were told that the management had no money to pay their salaries. Many were transferred to the positions of cleaners with a salary of 8,000 rubles.
“The head physician decided that we still need to cut and turn into cleaners for 8 thousand,” says Rusotskaya. – This was the last straw, and we began to write to the ministry, the prosecutor’s office, and everywhere, so that someone would at least take care of us, to look at the increase in the volume of work, that there was no recalculation of salaries, that medical examinations were not paid, that medical suits were not issued. Everything is at our expense.”
After the second wave of layoffs in the hospital, instead of 160 junior nurses and nurses, only 22 remained. An exorbitant burden fell on the remaining nurses, says Rusotskaya. A 12-hour work shift has to be served simultaneously at several posts, including intensive care units.
Employees suggested increasing the number of staff, including hiring nurses, providing nurses with breaks, fixing overtime payments, and stopping punishing union members.
After the nurses’ letter was circulated in the media, a commission from the Ministry of Health of the Moscow Region arrived at the hospital and has been conducting an inspection for several days. In addition, the center is checked by the prosecutor’s office and the State Labor Inspectorate.

Andriy Konoval, head of the independent trade union of medical workers Deystviye, where the nurses turned for help, told The Insider that before the hunger strike, no complaints helped the officials.
“For six months, nurses received replies and refusals for all appeals, there were not even real checks with on-site visits from any of the authorities. At best, the answers were compiled on the basis of information from the employer, ”Konoval notes.
The administration of the perinatal center, warned about the visit and inspection by the Ministry of Health, made efforts to quickly restore the marafet and even managed to abruptly change the torn sofa in the emergency department for a new one. All other changes only complicated the work of the medical staff.
The hospital management has practically stopped the work of the center, and the nurses are now constantly monitored by “guards” who record their every action, the doctors say.
“This is our leadership — someone from the accounting department, someone from medicine — they go and write down every five minutes where they went, where they went, where they took the woman, and so on. Now there are few women, they are watching to write that we are not working and just sitting,” says Rusotskaya.
According to her, Tatyana Shavrak, the head physician of the CPC, is now artificially holding back the admission of patients to the maternity hospital.
“She increased childbirth every month and recruited everyone, even with a temperature,” Rusotskaya said. And she is paid extra for this, not us. That is, if the center performed 450 births, and not 430, then the head physician receives incentive payments from the Ministry of Health. It does not affect our salary. The main thing was not quality, but quantity to take. Now everything has stopped. 1-2 people arrive at childbirth and the same number for caesarean. Before the check, we received from 28 to 32 people a day, caesarean 8-10. Now the resuscitation is without people.
Cleaning up documents on coronavirus patients
According to employees, before the check they even worked with covid patients, only they were not given the extra payments promised by the president for this – if there is only one polyclinic for the district, then it is not converted into a covid clinic, which means that it is very difficult for doctors to get coronavirus payments, although such patients are admitted . This also applies to maternity hospitals. According to junior nurses, covid patients were admitted to increase the number of births, for which the clinic management received bonuses. And after the situation received public publicity, all the documents on these patients began to be cleaned up.
“At the moment they are hiding them after we wrote to the Ministry of Health that there were a lot of coronavirus patients and they don’t want to pay us for them. Why don’t you follow the laws and instructions of Putin and Mishustin? The cards of people who were lying with the coronavirus began to be cleared — who left home, who were ordered to be transported, the main thing is that “we didn’t know anything”, so that everything was secret and hidden,” says the nurse.
Natalya Trukhina in a conversation with RBC confirmedthat the main requirement remains the expansion of staff in order to reduce the burden on health workers.
“The main requirement is that they add staff, because it’s really impossible to work like that, so that they pay a normal salary. Today I only had 15 minutes in a 12-hour day [перерыва]. Officially, I have two breaks: 15 and 30 minutes – for lunch and for tea. But that doesn’t happen. This situation began after the reduction of nurses in 2019 and in 2021. Now we perform the duties of both nurses and nurses. Moreover, my salary did not increase, but even fell. Previously, 40-42 thousand rubles came out. Now I have a good month, with the holidays, the maximum will be 30-32 thousand rubles,” she said.
